VHSL Physical/Parental Consent Form - New
New Physical for 20212.pdfThis is the newly revised VHSL Physical/Parental Consent Form that must be submitted to the coach before an athlete can participate in conditioning programs or before an athlete can try out for a sport. The day recorded by the examining physician on page 3 must be May 1, 2021 or later to be valid. STUDENTS AND PARENTS MUST SIGN WHERE INDICATED ON ALL FOUR PAGES!
